Subcontractor for NSC Per Capita Case Management Programs

NSC is currently recruiting contract case managers to serve immigrant victims of human trafficking and/or unaccompanied minors. Contractors typically serve an average of 1-3 cases per month which translates into about 10-20 per month.
Summary of Responsibilities:
1. Social service case management to victims of trafficking, including both pre-certified and certified clients. This includes:
- Intake, needs assessment and service planning
- Orientation of new clients
- Securing resources and services related to safety, housing, health, legal, social, employment and other needed services
- Administration of direct service funds including all applicable recordkeeping
- Communicating with third parties, such as employers, law enforcement and others
- Maintaining accurate, timely and orderly case and database records
- Advocating on behalf of the clients
2. Social service case management to unaccompanied alien minors. This includes:
- Comprehensive home safety and suitability assessment
- Securing resources and services related to safety, housing, health, legal, social, employment and other needed services
- Communicating with third parties as needed
- Maintaining accurate, timely and orderly case and database records
3. Prepare reports as needed including monthly case management reports, home study assessment and home visit reports.
4. Conduct all activities within the funders’ and NSC’s guidelines.
5. Actively participate in weekly supervision sessions and other department, agency, community and provider meetings as needed.
Qualifications:
- Bilingual skills are required (Spanish).
- Bachelor’s degree required. Master’s preferred.
- Some case management experience is preferred.
- Strong computer skills are required, especially WORD and EXCEL.
- Must have willingness to travel within 100 miles of Philadelphia.
- Knowledge of issues within the immigrant and refugee community is required.
- Applicant must be willing to work as a team member and have excellent interpersonal and cross-cultural communication skills.
